SPECIMENT
Portion of large bowel.

GROSS DESCRIPTION

Speciment labeled portion of bowel consists of a portion of large bowel measuring 10 cm. in length and up to 3.5 cm. in diameter. The attached periolic fat and part of omentum measures 4.5 x 3.8 x 1.9 cm. The bowel has already been opened and no orientation of the speciment is provided. On sectioning the constricted central part of the bowel is flat, ulcerated and constricted and measures about 1.8 cm. in diameter. Greyish white tissue is seen replacing the bowel wall and extends into the periolic fat. The bowel wall is firm and thickened up to 0.8 cm. in this area. The surrounding part of the bowel appears to be unremarkable. Recesction margins are submitted in cassette #1. Representative sections from the constricted part submitted in cassette #2 and #3 and from the rest of the bowel wall submitted in cassette #4. No lymph nodes identified in the pericolic fat and omentum. Representative sections submitted in cassette #5 and #6.

MICROSCOPIC EXAMINATION

Multiple sections show presence of well to moderately differentiated adenocarcinoma of large bowel. The tumor has invaded all layers of the bowel and extends into the pericolic fat. No evidence of vascular or perineural invasion seen. Tumor involvement of the lymph nodes could not be assessed because no lymph nodes were identified in the periolic fat. Random sections from the rest of the bowel are unremarkable. Proximal and distal lines of resection are free of tumor involvement. According to the modified Dukes' classification, Dukes' Stage B2 tumor.

DIAGNOSIS

Large bowel, well to moderately differentiated adenocarcinoma of colon. Dukes Stage B2.
